Final Report Forms & Instructions:
If you received a Project Support award prior to the September 2006 deadline, download the forms and instructions below to complete your final report form. If you received an award for the September 2006 deadline or later, login to your account at GOArts.com to complete your final report.

Your final report is due 30 days after the completion of a grant. GCAC must approve the final report before releaseing final payment on a grant.

Technical Assistance:

No application form is required; instead the organization must send a proposal letter, which defines the need and a specific plan for meeting that need, as well as a detailed budget. The Grants Program staff will review the proposal and with the approval of the Vice President for Programs and the President, award or decline grants based on the merits of each case. Technical Assistance grants are awarded as funding is available from the city of Columbus.

Advance Payment Request Form

Once the grantee's activity has officially begun, grantees may use the Advance Payment Request Form to request the first half of the grant payment, unless there are additional payment restrictions as specified in the grant agreement. GCAC must approve the Advance Payment Request before releasing payment on a grant. The form for the Advance Payment Request may be obtained below.
